Form 13: Financial Statement (Support Claims)

A Form 13: Financial Statement (Support Claims) is required if you are making or responding to a claim for child or spousal support in Ontario. It will need to be commissioned and provided as part of your Ontario family law case. Neighbourhood Notary can help you commission your completed form.

What is it? 

​In Ontario, if you are making or responding to a claim for child or spousal support or a claim to change support, a Form 13: Financial Statement (Support Claims) must be completed. The form may not be completed if your only claim for support is a claim for child support in the table amount under the Child Support Guidelines.

​The form should NOT be used if a claim is being made regarding property or debts. Instead, a Form 13.1: Financial Statement (Property and Support Claims) should be completed. Read more about Form 13.1.

Financial Statement / Disclosure

A Form 13: Financial Statement (Support Claims) will help you give the other party and the Court information about your finances, including your:
  • Income (how much you make).
  • Expenses (how much you spend on things like rent and childcare).
  • Assets (how much property or other valuables you own).
  • Debts (any money you owe).

Once your Form 13 has been filled out, Neighbourhood Notary can help you commission it. 

Thereafter, your Form 13 should be served on the other party (via Affidavit of Service) and also filed with the Court.
