United States Medical Licensing Examination (USMLE) Step 3 Certification of Identity (CID) Form

If you are applying for a licence to practice medicine in the United States you will be required to complete the United States Medical Licensing Examination, which is a three-step examination program for medical licensure in the United States. To complete Step 3, you need to submit your Application. As part of the Application, you will need to submit a notarized Step 3 Certification of Identity (CID) Form. What is an United States Medical Licensing Examination (USMLE) Step 3 Certification of Identity (CID) Form?
If you are applying for a licence to practice medicine in the United States you will be required to complete the United States Medical Licensing Examination (USMLE) as administered by the Federation of State Medical Boards and the National Board of Medical Examiners, you will be required to complete a three-part examination program. 

Step 3 requires an application to be completed. Step 3 requires a Step 3 Certification of Identity (CID) Form to be completed and notarized.

Once completed, your Step 3 Certification of Identity (CID) Form will need to be notarized before a notary public / commissioner of oaths.
