Royal College of Dental Surgeons of Ontario (RCDSO) Certificate of Registration Application

Your completed RCDSO application for a certificate of registration will need to be notarized, and certified true copies of supporting documents will need to be made. Neighbourhood Notary can help you notarize your application and make certified true copies.

​What is it? 

In order to practice dentistry in Ontario, you must be registered with the Royal College of Dental Surgeons of Ontario (RCDSO). 

To register with the RCDSO you must apply for a certificate of registration (licence) by submitting an application. The completed application must be approved by the RCDSO in order for you to be registered to practice dentistry in Ontario. 
Certificate of Registration 

Certificate of Registrations are available for many types of dental licences. A different application must be completed depending on the type of registration you are applying for: 
  • General (general dentistry practitioners)
  • Reinstatement
  • Academic (academic appointment at professional level) 
  • Speciality
  • Education (hospital interns and residents)
  • Graduate Student (postgraduate and graduate students)
  • Academic Visitor (short-term course instructor)
  • Post-Speciality Training
  • Short Duration

Why Do I Need A Notary Public? 

After you have filled out / completed the application form the RCDSO requires that your application to be notarized before a notary public. As well you may be required to provide certified true copies of supporting documents (i.e. diploma, passport, etc.).
