Medical Council of Canada (MCC) Certified Identity Confirmation Form

If you are applying to take your Medical Council of Canada (MCC) qualifying exams to become a Licentiate of the Medical Council of Canada (LMCC) you will need to submit a notarized copy of a Certified Identity Confirmation Form as well as a notarized identity document. Neighbourhood Notary can help you notarize it.

The Medical Council of Canada (MCC) Qualifying Examination

The Medical Council of Canada (MCC) provides a standardized assessment of physicians in Canada via a qualifying exam known as the Medical Council of Canada Qualifying Examination (MCCQE) Part I. As of June 10, 2021, a second qualifying exam known as the MCCQE Part II was cancelled. 

Graduates of accredited Canadian medical schools, medical schools listed in the World Directory of Medical Schools, or United States Schools of Osteopathic Medicine accredited by the American Osteopathic Association, may complete the MCC examinations.

The MCC will grant a qualification in medicine known as the Licentiate of the Medical Council of Canada (LMCC) to physicians who have met its requirements. The LMCC is NOT a licence to practice medicine (this responsibility belongs to the provincial and territorial Medical Regulatory Authorities).

This is a way of ensuring a licensing standard for independent practitioners in Canada (find out more information on this licensing pathway here).

​Certified Identity Confirmation Form


​In conjunction with your application package for MCC examinations, a completed copy of the Certified Identity Confirmation Form is required to be submitted in order to prove your identity, and to confirm your qualifications. The form must be certified/notarized by an authorized individual (such as a notary public). The certifying individual must also see and photocopy your passport or other original documents and certify the photocopies. Neighbourhood Notary can assist with notarizing your Certified Identity Confirmation Form and photocopies of your identity document.
