Ontario Lawyer or Paralegal Application for Surrender of Licence to Practise Law Under By-Law 4

If you are applying to the Law Society of Ontario to surrender your licence to practice law under By-Law 4 as a lawyer or paralegal, you will need to have your application commissioned befefore submitting it. Neighbourhood Notary can help you commission it.

Surrender of Licence to Provide Legal Services

Lawyer or Paralegal licensees who no longer wish to provide legal services in Ontario may apply to the Law Society of Ontario (Law Society) to surrender their licence. Applicants are encouraged to review By-Law 4 for information about surrendering their licence.

For more information on how to surrender your licence, as well as to download the lawyer or paralegal application form, check out the Law Society's website. 
Required Documents 

You will be required to submit a number of documentation to the Law Society as part of your application: 

1. Completed application form.
2. Signed insurance release form (for lawyer's this would be from LAWPRO). 
3. Proof of trust/mixed trust/estate accounts closure or Proof of no signing authority over all trust/mixed trust/estate accounts.
4. Outstanding Annual Report Filings. 
5. For lawyers, you will need to advise of the location of active and/or closed files, wills, powers of attorney, corporate seals and corporate minute books.

Prior to submitting your application to the Law Society, you will have to have your application commissioned. You will need to declare that the information provided by you with respect to the application and in the documents provided in connection with your application is true, accurate, and complete. Neighbourhood Notary can help you commission the form.
