The Form 2016 - Statutory Declaration form is used for the purpose of declaring the existence of a common-law relationship under the Public Service Superannuation Act (PSSA) to Public Works and Government Services Canada (PWGSC) and to the Public Service Pension Centre. The declarant will need to include the following information in their Form 2016 - Statutory Declaration: their full name, the city and province they reside in, the name of their common-law spouse or partner, and the date (i.e. year, month, day) since they have been together. |
Commissioning your Form 2016 - Statutory Declaration
Once your Form 2016 - Statutory Declaration is completed, the common-law partner who is submitting the form to the Public Service Pension Centre must declare that the information contained in the statutory declaration is true and correct. This must be done before a commissioner of oaths or notary public.
