What is it? 

If you are applying for student financial assistance (loans and bursaries) in Québec, you may have to submit a Form 1076: Solemn Declaration as part of your application. 

A Form 1076: Solemn Declaration must be filled out if you need to clarify your situation or are unable to provide one or more of the required documents (i.e. declaration of father or spouse).

Who Can Apply for Student Financial Assistance in Quebec? 

In order to apply for government assistance, you must meet the following requirements:
  • Be a Canadian citizen or have legal status in Canada 
  • Be a Québec resident or deemed to reside in Québec
  • Be admitted to a recognized educational institution and a be pursuing full-time studies in a recognized program
  • You must not have exceeded the number of months of eligibility for which financial assistance may be awarded 
  • You must not have reached the debt limit for your level of education, type of degree or program 
  • You do not have sufficient financial studies to pursue your studies

Commissioning your Form 1076: Solemn Declaration

You must complete the Form 1076, and then have it commissioned by a commissioner of oaths. The form will not be considered valid unless it has been signed by a Commissioner for Oaths.

Note that by having it commissioned by a commissioner of oaths you are swearing that the contents of your document are true, and there may be legal consequences if the contents prove false.  ​
