Commissioning your Form 1076: Solemn Declaration
You must complete the Form 1076, and then have it commissioned by a commissioner of oaths. The form will not be considered valid unless it has been signed by a Commissioner for Oaths.
Note that by having it commissioned by a commissioner of oaths you are swearing that the contents of your document are true, and there may be legal consequences if the contents prove false.
